Government healthcare plans

The government replaced EPC plans with Chronic Disease Management (CDM) plans. Most recently, these were updated into a single GP Chronic Condition Management Plan (GPCCMP).
However, most patients and clinics still call them EPC plans.

GP Chronic Condition Management Plan

A GP Chronic Condition Management Plan (GPCCMP) is a referral that is written by us which provides eligible patients with Medicare-subsidised allied health appointments, like physio and podiatry.

To be eligible for an EPC or GPMP, a patient must have a chronic or terminal medical condition with or without complex care needs.
A chronic health condition is defined as one that has been present for 6 months or more. It includes, but is not limited to, asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke.

You will need to assess your eligibility for an EPC plan.

To obtain an EPC or GPMP you need to make a standard appointment with us at Mallee Border Health Centre.
Please note that this referral takes 20 minutes, and cannot be done in a standard appointment when other issues are being discussed. If you are coming to see Di about another issue, you will be required to book a separate standard appointment.

To ensure continuity of care, it is important that you continue to visit your usual GP/NP for your initial EPC plan and any reviews of your EPC plan.

REFERRAL is bulk billed.

The EPC ALLIED HEATH PAYMENT is billed to the patient at the end of the consult.

This is then rebated to Medicare and a percentage is refunded to the patients Bank account.

Integrated Wellbeing
Service

An Integrated Wellbeing Service (IWS) is a referral that is written by us at Mallee Border Health Centre which provides eligible patients with Medicare-subsidised allied health appointments, like physio and podiatry.

It supports patients who require allied health or specialist nurse intervention to understand and manage their chronic disease conditions.

The plan is funded by Country SA Primary Health Network and In partnership with FocusOne Health, Mallee Border Health Centre delivers the Integrated Wellbeing Service (IWS) to the Mallee.

To be eligible for an IWS plan you will need to assess your eligibility. To obtain an IWS you need to make a standard appointment with us at Mallee Border Health Centre.
Please note that this referral takes 20 minutes, and cannot be done in a standard appointment when other issues are being discussed. If you are coming to see Di about another issue, you will be required to book a separate standard appointment.

After the referral appointment at the Mallee Border Health Centre and it is signed off by Dr Peter Hamilton and then a FocusOne Care Coordinator will then assess your eligibility for the services, based on your circumstances.

Health Assessments

Preventative Health Assessments are a health assessment carried out at 45-49 years of age and another at 75 years and over.

The 75+ health assessment is a comprehensive Medicare-funded annual check for people aged 75 years and over, designed to review overall health, prevent complications, and support ongoing wellbeing.

The 45-49 Year Health Check

Your mid to late 40s can bring on its own set of health challenges. However, a Medicare-subsidised 45-49 year-old health assessment can help Australians identify potential chronic conditions early and reduce the risks.

What’s included

The health check is a comprehensive assessment of tests and examinations to determine your risk of chronic diseases. It also helps you make healthy lifestyle changes to prevent or delay your risk of disease.

Assessment items

As part of the health assessment, we’ll ask you about the following:

  • Lifestyle (diet, exercise, smoking, and drinking).
  • Signs of skin cancer.
  • Mental health condition.
  • Preconception care.
  • Family history of chronic disease.

What it involves

We may also perform the following:

Measure your:

  • Weight and height.
  • Blood pressure.
  • Cholesterol.

And assess the need for a:

  • Pap test.
  • Mammogram.
  • Vaccinations.
  • Testicular examination.
  • Prostate assessment.
