Enhanced Primary Care Plans (EPC)

An Enhanced Primary Care Plan (EPC) is a referral that is written by us which provides eligible patients with Medicare-subsidized allied health appointments, like physio and podiatry. An EPC may also be referred to as a GP Management Plan (GPMP).

WHO IS ELIGIBLE?

To be eligible for an EPC or GPMP, a patient must have a chronic or terminal medical condition with or without complex care needs.
A chronic health condition is defined as one that has been present for 6 months or more. It includes, but is not limited to, asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke.

You will need to assess your eligibility for an EPC plan.

HOW DO I GET ONE?

To obtain an EPC or GPMP you need to make a standard appointment with us at Mallee Border Health Centre.
Please note that this referral takes 20 minutes, and cannot be done in a standard appointment when other issues are being discussed. If you are coming to see Di about another issue, you will be required to book a separate standard appointment.

To ensure continuity of care, it is important that you continue to visit your usual GP/NP for your initial EPC plan and any reviews of your EPC plan.

PAYMENT
REFERRAL is bulk billed.
The EPC ALLIED HEATH PAYMENT is billed to the patient at the end of the consult. This is then rebated to Medicare and a percentage is refunded to the patients Bank account.

Integrated Wellbeing Service (IWS)

An IWS is a referral that is written by us at Mallee Border Health Centre which provides eligible patients with Medicare-subsidized allied health appointments, like physio and podiatry. It supports patients who require allied health or specialist nurse intervention to understand and manage their chronic disease conditions. The plan is funded by Country SA Primary Health Network and In partnership with FocusOne Health, Mallee Border Health Centre delivers the Integrated Wellbeing Service (IWS) to the Mallee.

HOW TO GET ONE?

To be eligible for an IWS plan you will need to assess your eligibility. To obtain an IWS you need to make a standard appointment with us at Mallee Border Health Centre.
Please note that this referral takes 20 minutes, and cannot be done in a standard appointment when other issues are being discussed. If you are coming to see Di about another issue, you will be required to book a separate standard appointment.

After the referral appointment at the Mallee Border Health Centre and it is signed off by Dr Peter Hamilton and then a FocusOne Care Coordinator will then assess your eligibility for the services, based on your circumstances.

 – Exercise Physiology
 – Care Planning and Coordination
 – Healthy Eating Programs 

PAYMENT
REFERRAL is bulk billed.
IWS HEATH PLAN PAYMENT is billed direct to the plan, patient is not billed.
